Consent forms for RT and SACT

Informed consent is a cornerstone of ethical and patient-centred cancer care. At the PMGH Cancer Centre, we require all patients undergoing radiotherapy (RT) or systemic anti-cancer therapy (SACT) to complete a formal consent process before treatment begins.

 

Our consent forms are designed to:

  • Clearly explain the purpose, benefits, and potential risks of treatment

  • Support patients in making informed decisions about their care

  • Document that discussions have taken place between the patient and treating clinician

  • Ensure legal and professional accountability in line with national and institutional policies

 

Each form is tailored to the specific treatment modality and cancer site, and includes space for:

  • Treatment intent (curative vs. palliative)

  • Potential short- and long-term side effects

  • Patient and witness signatures

  • Interpreter notes (if applicable)

 

Patients are encouraged to ask questions, take time to consider their options, and involve family members in the decision-making process where appropriate.
