Consent forms for RT and SACT
Informed consent is a cornerstone of ethical and patient-centred cancer care. At the PMGH Cancer Centre, we require all patients undergoing radiotherapy (RT) or systemic anti-cancer therapy (SACT) to complete a formal consent process before treatment begins.
Our consent forms are designed to:

Clearly explain the purpose, benefits, and potential risks of treatment

Support patients in making informed decisions about their care

Document that discussions have taken place between the patient and treating clinician

Ensure legal and professional accountability in line with national and institutional policies
Each form is tailored to the specific treatment modality and cancer site, and includes space for:

Treatment intent (curative vs. palliative)

Potential short- and long-term side effects

Patient and witness signatures

Interpreter notes (if applicable)
Patients are encouraged to ask questions, take time to consider their options, and involve family members in the decision-making process where appropriate.
